Please use the following procedure for downloading and completing the application:

Paper Copy

1. Download the Rite Care application (PDF, 133.44 KB)

2. CLICK the link above, Adobe will open the application.

3. CLICK the download icon - upper righthand corner.  See below for a picture of the download icon. 

4. Save the file to your desktop or someplace you'll easily find it.

5. Open the file and print. 

6. Fill in the document by pen, following the instructions on the document

7. After filling in the application, please send it to the following address:
